Coroner's Release to Cremate:
Funeral Homes/Funeral Directors seeking a "Coroner's Release to Cremate" for a decedent should complete the form below, or directly contact the Coroner or Deputy handling the investigation. Please make sure to attach the completed burial transit permit (BTP) and cremation authorization form signed by the family/responsible party.
The form will be emailed to our staff, your request will be reviewed, and we will respond promptly within appx 24 hours.
Funeral Home Decedent Release:
Please contact our main office or the Deputy Coroner directly for inquiries related to the release of the decedent.
Assigning Death Certificates in DRIVE:
When submitting a death certificate to our office within DRIVE please "Refer to Coroner" and assign the Coroner or Deputy Coroner listed on the Burial Transit Permit.
